Description

In the face of rising global migration due to war, famine, work, and pastoralism, the lack of, and fragmentation of healthcare systems often fail to meet the needs of mobile populations, especially in the global south.

This creates barriers to accessing essential medical services, hindering continuity of care, disease surveillance, and healthcare administrative efficiency.

IntelliSOFT, a health IT leader in sub-Saharan Africa, is committed to leveraging technology to address some of the most pressing health system challenges. In this quest, IntelliSOFT has developed the Cross-Border Digital Health System abbreviated CBDHS. The CBDHS interconnects health information systems in health facilities enabling applicable data sharing across facilities in cross-border areas while providing enhanced linkage with local, national, and regional data warehouses such as DHIS2.

CBDHS achieves this through a two-pronged approach. First, enhancing data collection with simple FHIR-based tools. Secondly, using open-source and standards-based healthcare data exchange standards enables data sharing between separate countries.

The CBDHS system consists of two major components; a centrally-hosted regional OpenHIE-based Health Information Exchange (HIE) Layer and FHIR-based data collection tools.

Imagine a central hub connecting multiple national EMRs within, and across borders. This layer (the HIE) utilizes standardized protocols and interoperability services to facilitate seamless data exchange, ensuring medical records travel alongside migrants.

The FHIR-based data-collection tools are complementary to the HIE. Capable of working independently and offline, they enhance EMRs with cross-border functionalities. They facilitate clinical data capture, reporting tailored to migrant healthcare needs, empowering healthcare providers to properly administer healthcare leveraging the benefits of a digital health system.
When the FHIR-based data collection tools are connected to the HIE, more power is unlocked by providers, they can view previously captured clinical records and seamlessly share data.

With the ever-expanding landscape of EMR systems, the CBDHS system offers a vendor-agnostic FHIR-API enabling EMRs to plug into, providing a flexible, scalable approach for easy integration of capabilities over time.

Notable immediate benefits provided by the CBDHS are:

Improved continuity of care: Mobile populations can access their medical history, empowering healthcare providers to deliver informed, timely care.
Enhanced disease surveillance: Real-time cross-border data-sharing strengthens public-health responses to outbreaks and emerging threats.
Empowered providers: Access to their health records informs decision-making and participation in the client's healthcare journey

Even with these advancements, one major challenge still experienced is the need for cross-border data-sharing policies in the region. Ensuring health data security, privacy, and compliance with data protection regulations, becomes complex when dealing with multiple jurisdictions.

Efforts are underway within several regional bodies including IGAD, the East African Community, and the African Union on establishing standardized, transparent cross-border data-sharing protocols addressing security, privacy, and regulatory compliance concerns across jurisdictions. Engaging in international dialogue and forming consensus-driven agreements will help overcome challenges associated with cross-border data-sharing in the CBDHS context.

The CBDHS presents a groundbreaking opportunity to advance health equity for migrants. We invite collaboration among policymakers, healthcare professionals, and technology developers to refine and implement this framework, building a future where migration doesn't impede one's right to quality healthcare.
